This offer includes; dental examination (011 or 012), scale and clean (114), fluoride (121), intra-oral x-rays (022 x 2) and unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the Practitioner.
Where eligible, benefits for services provided will be deducted from the patient’s available health fund limits.
If patient does not have available health fund rebates (including but not limited to situations such as limit reached, item not covered, waiting period), patient can be charged up to the adult ($250) or kids ($140) dental check-up and clean package fee to make up for the shortfall.

The patient can at any time choose to not have any part of the above treatment plan and the relevant codes will not be processed as part of the appointment. However, patients who choose not to have certain parts of the treatment also accept the risk that comes with not having the complete dental check-up.

The Take Home Whitening promotion covers individual whitening/fluoride tray (926 x 2), impression/instruction-home whitening (119×2) and the whitening gel (927) deemed appropriate for the patient in the clinical opinion of the practitioner. This may be PolaNight 18% (Carbamide Peroxide), and/or PolaDay 6% (Hydrogen Peroxide) or Opalescence Boost 16% or other equivalent reputable product. This can come in different measurement syringes.
